This page explains what the PayVariance rate-check form collects, how that information is used and stored, who it may be shared with, and how to ask us to access, correct, or delete it. PayVariance is not a law firm and does not provide legal advice.
Who operates this site
This site is operated by PayVariance. For any privacy request or question, contact us at hello@payvariance.com. The named legal operating entity and a physical mailing address will be identified here before the service accepts paid engagements.
What we collect
The rate-check form asks you to submit:
- practice name;
- NPI (National Provider Identifier — a public identifier);
- the payer or health plan you want checked;
- your name;
- email address;
- phone number (optional).
These are personal identifiers, not patient or clinical data. Do not send patient or claim data through this form or by email. If a later remittance review is appropriate, we will first define the required data, the transfer method, and the handling terms in a separate written agreement.
In addition, our hosting and security provider may automatically process standard technical request data — such as your IP address, request timestamp, browser user-agent, and referring page — to operate and secure the site. We use this only for operation, security, and abuse prevention, not to build a profile of you.
We do not use advertising or analytics cookies.
How we use it
- to fulfill the rate check you requested;
- to communicate with you about that request;
- to operate, secure, and maintain the site;
- to comply with applicable law.
We do not use your information for unrelated marketing without your consent.
Who we share it with
We disclose form submissions only to the categories of service providers and recipients needed to run the service:
- site hosting and form processing (our hosting platform);
- database storage (Supabase);
- email delivery and forwarding for our contact address;
- professional advisers under confidentiality; and
- disclosures required by law.
We do not sell personal information or disclose it for cross-context behavioral advertising. We disclose form submissions only to service providers that host the site, store submissions, or deliver requested communications; to advisers under confidentiality; and when required by law.
Do Not Track and cross-site tracking
This site does not use advertising or analytics technology to track visitors over time and across third-party websites. Because we do not engage in that tracking, browser Do Not Track signals do not change how this site operates. We do not knowingly allow other parties to collect personally identifiable information on this site for cross-site tracking.
How long we keep it
We keep rate-check submissions only as long as needed to handle your request and for a limited follow-up period afterward — up to 24 months from submission — after which we delete them, unless you ask us to delete them sooner or a longer period is required by law. Deletion requests are actioned through the process below.
Access, correction, and deletion
You may ask us to tell you what we hold about you, to correct it, or to delete it. Email hello@payvariance.com from the address you submitted, or describe the submission, and we will act on the request.
Security
We limit access to submissions to the people and services that need it and apply reasonable safeguards. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
Changes to this policy
If we make a material change, we will update the "last updated" date above and post a prominent notice on this page, and give direct notice where appropriate.
This policy describes current practices and will be reviewed by counsel before the service accepts paid engagements.
